Frank P. Phillips Memorial YMCA is committed to providing the highest level of security to members. Personal information, payment information and other data transmitted through our system are never shared or sold.
Membership Waiver
Frank P. Phillips Memorial YMCA is a charitable, non-profit membership organization. I understand the YMCA will have no liability or responsibility for any personal injuries, or loss or damage to personal property, sustained by the member(s) while using the YMCA facilities. I understand membership card(s) must be presented to enter the facility. Loaning membership cards to another individual is prohibited and will subject member(s) to loss of membership privileges.
Memberships are not transferable. I understand if my membership lapses for 30 days or more, joining fees, in addition to membership dues and any and all outstanding balances will be assessed upon rejoining.
As a member of the YMCA you are agreeing to follow the policies, procedures, and appropriate behaviors of the safety and comfort of all members and guests.
I have read and understand the Membership Waiver. In addition I understand and agree that the conditions of the Membership Waiver are in effect throughout my entire membership with Frank P. Phillips Memorial YMCA.
Electronic Funds (EFT) or Credit Card Authorization
I give authority to have pre-authorized payments drawn by Frank P. Phillips Memorial YMCA on my credit card or bank account for membership payments. Membership dues paid by credit card or bank account are continuous and can be cancelled only by submitting a YMCA cancellation form (available from the YMCA Membership Director) AT LEAST 30 DAYS PRIOR TO CHARGE DATE. Please check your credit card statement to make sure your account is not charged after the effective termination date. The YMCA Board of Directors may, at its discretion, adjust the monthly rate applicable to my category of membership. I understand that I will receive at least four-weeks notice prior to any such change. Should my credit card institution for any reason not honor my membership charges, I understand that I am still responsible for that payment, plus any service charge applied by the YMCA. This is in addition to any service fee my credit card institution may charge.
Photo/Talent Release
I hereby irrevocably release, consent and allow Frank P. Phillips Memorial YMCA and its agents to use my photograph/likeness/voice, as it pertains to my participation with the Y, in any manner for promotion efforts without expectation of any reimbursement in connection with its use.
